Guidance on Child Assent for children ages 8-17

The Case IRB suggests that researchers utilize a written and signed child assent form for subjects in this age range (in addition to parental permission). When drafting a written assent form, researchers may find it helpful to create a simplified version of an adult informed consent document, appropriate for the reading level of the subjects (see "Example Informed Consent Document" above).

Case IRB Responsible Investigator Certification Form

Only Case faculty members who meet the human subjects research education requirement may act as Responsible Investigators, unless certified as someone, "with whom the university has or intends to have an ongoing contractual relationship". Please open, print and complete the form below; and, if applicable, send it to the Case IRB Office before or along with a completed new protocol application. Once accepted by the Case IRB Office, there is no need to complete this form again.
