Procedures to Obtain a Letter of Exemption
Examples of experiments that are categorized as exempt
Investigators needing a letter of exemption for their recombinant DNA experiments:
The following form will need to be completed and sent to the Case Institutional Biosafety Committee Office in order to process the application.
This form will be reviewed by the Case IBC Director and the Chairperson or Co-chairperson. After approval of the Exemption status, a
letter of exemption will be forwarded to the Principal Investigator of the application.
The following form can be saved and completed on your local computer. To expedite this procedure the form can be e-mailed as an attachment
to the Case IBC .
However, to complete this process a printed and signed form must be forwarded to the IBC office (address is located on the form or website).
Please maintain the format of the form.
